The document is written. Leadership signs off. Training is scheduled.

The organization feels like governance is now in place. But that is usually the point where a different problem starts.

The real challenge is not policy approval. It is policy operationalization.

That distinction matters more now because the global direction of AI governance is increasingly clear. In the US, NIST frames governance as policies, processes, procedures, and practices. In the EU, the AI Act points toward risk management, documentation, human oversight, and monitoring for certain systems. In the UK, the model emphasizes accountability, governance, transparency, and contestability. OECD guidance also reinforces the idea that responsible AI requires embedding expectations into management systems and tracking results over time.

In other words, policy alone is not the destination. It is the starting point.

Three signs your AI policy is approved but not operational

1. There is no intake path

If teams can start using AI tools or AI-enabled workflows without a standard path for intake, review, or request, governance is already behind the business.

This is one of the earliest and most common breakdowns.

The policy may say the right things, but the organization still has no practical way to identify what needs review.

Action step:

Ask whether every new AI use case has a clear entry point.

If not, that is one of the first controls to build.

2. There is no clear owner

A policy often says “the organization,” “management,” or “the business” is responsible.

That is not enough.

Someone has to own review. Someone has to approve or reject. Someone has to manage exceptions. Someone has to follow up when use changes.

Without named ownership, accountability stays broad and operational responsibility stays vague.

Action step:

Take one policy statement and assign:

  • one owner
  • one reviewer
  • one escalation point

3. There is no evidence trail

This is where many governance programs look stronger than they really are.

The organization may have a policy and a training deck, but can it show:

  • what was reviewed,
  • what was approved,
  • what was restricted,
  • what was escalated,
  • and what happened afterward?

If not, governance is difficult to demonstrate, defend, or improve.

Action step:

Pick one governed AI activity and decide what record should exist:

  • intake form
  • decision log
  • exception note
  • review outcome

A simple operational test

Take one sentence from your AI policy and ask:

  • Who owns it?
  • What workflow supports it?
  • What evidence proves it is happening?

That is a simple test, but it is powerful.

Because if the answer to those questions is vague, then the policy may be approved without being operational.

The practical takeaway

The goal of AI governance is not just to publish a good policy. It is to make the policy work in real operations.

That means translating policy into:

  • intake
  • ownership
  • review
  • evidence
  • monitoring
